Original Abstract

Human behaviour plays a key role in process industry safety. The possible actions that could be taken upon detecting an accident as well as how one reacts once they hear an alarm need to be considered. Many of major accidents were brought about by the alarm and trip system failure. Some of the most important accidents will be described in this contribution. Whenever risks are found to be unacceptable, solutions have to be found either by fundamental design changes or by providing protection measures and operators training. But the design of measurement, alarm and trip system should be appropriate to the premise or processes. Selection and evaluation of unsuitable scenarios can result in conservative designs that are not economical or designs with unacceptably high risks. The objective of this contribution is to consider the control room layout and measurement, alarm and trip system and propose risk reduction method of control room with alarm and trip systems orientation.
